Financial Information

To better serve you, the doctors of Arrowhead Women’s Center participate in many insurance plans.  If you are unsure if your insurance is one of those accepted, please call your insurance provider and inquire if are physicians are part of your insurance network.

Your insurance company informs all participants that it is ultimately your responsibility to know and understand your policy and its benefits.  This is important so you are not faced with any financial surprises.  If your insurance requires a referral, please be sure to request one from your primary care doctor before your visit.  We attempt to verify coverage before your visit with the information you provide. However, verification of coverage does not guarantee payment at the benefit level quoted.

As a courtesy to you we will bill your insurance company for your visit. If you have more than one insurance, it is important that you supply us with information from both insurances. Insurance companies require that we submit all claims within a specific time limit.  We do our best to follow all guidelines set forth by your insurance company.  However, if your insurance changes and you fail to inform us, we may be unable to bill the appropriate company within these time limits.  If we are unable to bill your insurance it is important to know that you are ultimately responsible for all incurred charges. For your benefit, please notify us of any changes as soon as possible, including any changes you may have in your address and phone number.

We are sure you understand that it is customary for payment to be made when services are rendered.  All co-pays, co-insurance, and deductibles are to be paid when you check in for an appointment (before being seen).  For your convenience we accept Visa, MasterCard, American Express and Discover credit cards, cash and checks.  As is customary, reasonable late fees or collection fees may be assessed in the event of late payment or non-payment of your balance.
